Forms

Every participant in our Laser Tag games must have a signed waiver form. For minors (under the age of 18) a parent or adult guardian must also sign the form. Please have signed forms ready for our Game Coach prior to the onset of Game Play.

For parties requesting M-rated games allowed. All players must fill out the form if they are to watch or even play the M-Rated games we offer.

Content is generally suitable for all ages. May contain minimal cartoon, fantasy or mild violence and/or infrequent use of mild language.

Content is generally suitable for ages 13 and older. May contain violence, suggestive themes, crude humor, minimal blood, simulated gambling and/or infrequent use of strong language.

Content is generally suitable for ages 17 and older. May contain intense violence, blood and gore, sexual content and/or strong language.
